Thank you for choosing West Georgia Sleep Disorders Center and Neurology Associates as your patients Sleep Disorder and Neurology provider.

To better serve you your patients and staff, we have developed a referral process that is intended to save time for you and your staff. Referrals are made by calling our central scheduling 770.832.2775, ext 14: indicating the preferred office location for appointment.
Due to the nature of our business office hours may fluctuate. As needed, leave a message and your call will be promptly returned. Physician offices may schedule appointment by completing and faxing a sleep study or neurodignositc order form to central scheduling, 770.832.9529. We ask that when faxing the order forms to our office to please include a copy of the patients insurance card.

The following is needed prior to appointment for our staff to pre-certify test or procedures that your patient may have at our facility.

1. All pertinent lab results from the last 12 months only.
2. Radiology reports pertinent to present problem only.
3. Copy of insurance cards, and referral number if needed (please fax ASAP).
4. Demographic information (please fax ASAP).
5. Sleep Study or Neurodiagnostic order form from the physician (please fax ASAP).


Please remind your patient that it is important to bring all of their medications in original bottles (including non-prescription) to their appointment with us. Pertinent medical records can be faxed to 770.832.9529, or the patient may hand deliver to our office.

Our appointment scheduler is available 8:00 a.m.to 4:00 p.m. Monday - Friday at 770.832.2775 ext. 14. Our entire staff is eager to assist you with all of your referral needs.
